An Evaluation of Systemic Vancomycin Dosing in Obese Patients

We retrospectively identified 67 patients with severe or morbid obesity (body mass index ≥35 kg/m(2)) who had received intravenous vancomycin at our institution. We observed that an initial dose of 45 to 65 mg/kg vancomycin per day based upon ideal body weight rather than actual body weight was more predictive of initial trough concentrations between 15 and 20 mcg/mL.

Methodological Study of Vancomycin Dosing in Elderly Patients Using Actual Serum Creatinine Versus Rounded Serum Creatinine

PURPOSE The practice of intentional rounding up of serum creatinine (SCr) in elderly patients with low measured values can lead to an underestimation of creatinine clearance and subsequent inaccurate dosing of medications. Thus, the purpose of this study was to evaluate the accuracy of vancomycin dose calculations for patients aged ≥65 years using an SCr rounded up to 1 mg/dL versus actual SCr....

Overweight and obese patients in a primary care population report less sleep than patients with a normal body mass index.

BACKGROUND Insufficient sleep and obesity are common in the United States. Restricted sleep causes important neurocognitive changes, including excessive daytime sleepiness and altered mood. This may result in work-related injuries and automotive crashes. Evidence links sleep loss to hormonal changes that could result in obesity. This article examines the association between restricted sleep and...

Optimizing Vancomycin Dosing in Obese and Morbidly Obese Patients with MRSA Infections

In the United States (U.S.), greater than one-third of the adult population over the age of twenty is classified as obese. Obesity accounts for 20% of individuals under the age of 20% and accounts for 40% of adults over the age of sixty, thus signaling an increase in rates of obesity with increased age [1]. In 1990, all states within the U.S. had a prevalence of obesity of less than 15%. Ten ye...
